I don't think Malak's head is actually in the game, since it was a premade cutscene, but I could be wrong...

just look in heads.2da for Malak ones. if ti isn;'t there, it isn;t in the game

Well there are models for then since we have models n_darthmalak, n_darthmalak02, n_darthmalak03 and n_jedimalek with I suspect are his head and body models as N_DarthMalakH01 is for his "Vader wannabe", N_jediMalekH01 for his "when I was a jedi and had a jaw" and N_jediMalekH02 for his "I am cleaning up my jaw" head textures.

I guess BioWare simply included the models and textures they used in the cutscenes in the game.
